Registration fees for the 2004 Annual Meeting are:

* The difference between the member and non-member registration fee is equal to the cost of becoming a member of SRNT ($120 for regular members, $35.00 for trainees). If you are a non-member and plan to attend the meeting, click here to fill out your online membership application or fax a completed membership application (34KB PDF) to SRNT. Please forward your CV along with your application.

Please complete all items on this secure online registration form. The deadline for pre-registration is February 9, 2004. After that date, plan to register at the meeting site. Print out one copy of this completed form for your records.

Payment by credit card must accompany each registration form. This online form is secure.
The deadline for inclusion in the Attendee Directory (distributed on-site) is January 26, 2004.

You will receive a confirmation e-mail within three weeks of submitting your registration form. If you do not receive a confirmation e-mail from the National Office, we did not receive your online submission.

Cancellation Policy
Any Annual Meeting registration cancellations must be made in writing directly to SRNT. If received before February 8, 2004, SRNT will apply a $50 administration fee, and refund the remainder of your registration fee after the meeting. After February 8, 2004, no refund will be given. Walk-ins and replacements are always welcome.

Registering for the Pre-Meeting Conferences
There are no registration fees for the premeeting conferences but space is limited and pre-registration is required. Use the registration form and check off the appropriate box for the pre-meeting conference.

Please note: Registering for any of the pre- conference workshops DOES NOT register you for the SRNT Annual Meeting. Complete the entire registration form, including payment, to register for the Annual Meeting.

All potential attendees of the SRNT Annual Meeting should be aware that an individual’s current or past employment history is not a criterion on which SRNT bases registration decisions. Thus, meeting attendees may include representatives of the tobacco industry.
